EMPOWER

Employ learning principals to improve participation

Support people with chronic diseases in self-care and social participation!

Participation is not a matter of course, but you can learn to do so.

People with chronic diseases and their nearest can learn to participate if it is not self-explanatory. Therefore learning principals need to be implemented in all of their supportive structures. Ownership and personal environment are key elements.

Mission of PPP

People with chronic diseases are able to lifelong participate by their own means in their personal environment and live at least five years longer in good health by 2040.
This PPP innovates and facilitates key stakeholders to share knowledge, expertise and best-practices in the transition from a medical model towards a social impact model.
