In this project, we are developing an innovative and social approach to foster positive behavior change in individuals living with diabetes. Diabetes places a significant burden on both the healthcare system and those affected by the condition. As the number of people with diabetes continues to rise rapidly, there is a pressing need for innovations in care, welfare, and tailored extramural support.

With the MEDIS project we are building on the successes of previous initiatives, "Herstelcirkel++" and "Technology Support for Diabetes (TSFD)". The “Herstelcirkel++” project identified that a peer support approach effectively motivates the target group. The TSFD project has provided insights in how to design and evaluate eHealth for people with diabetes. By integrating gained knowledge from both projects, our current project aims to enhance innovative and social support within the informal environments of individuals with diabetes to positively influence behaviour change related to lifestyle, self- care and overall health.

In this follow-up project, we will design and develop innovative services and technologies based on peer support and community building principles. Understanding that behavior change is complex, we recognize the importance of fostering a sense of connectedness and involvement among those who experience similar challenges. It is crucial that these interventions are implemented effectively and that their design aligns well with the daily lives of people with diabetes and the healthcare system.
