Early diagnosis of stable Angina uSINg Extracellular veSicleS (EASINESS)
This project aims to further investigate and develop an automated method for isolating and analyzing blood plasma extracellular vesicles of chest pain patients in order to discriminate patients with true underlying cardiac pathology from those without
The EASINESS consortium brings together UMC Utrecht’s cardiovascular and molecular research expertise and PERSUASIVE’s diagnostic product development and commercialization expertise. The consortium embarked on a journey to build an automated laboratory test method that uses biomarkers in blood-derived extracellular vesicles (EVs) to provide an early diagnostic test result. The work has its foundations in UMC Utrecht research on EVs and female and male-specific biomarkers for identification of chronic coronary syndrome (CCS) in chest pain patients.
Chronic coronary syndrome (CCS; previously named stable angina) is a very serious condition with a 5-6x higher risk of having a heart attack or even dying. Of chest pain patients seen by GPs (in the Netherlands over 500.000 per year) only ~10% will have CCS. The challenge for GPs is to determine among a large patient group patients who actually has a chronic heart condition or not. By isolating EVs from patients’ blood and examining their contents, we can identify presence of CCS to support GP referral. This is a huge gain in referral efficiency, a significant cost reduction and a great help for patients who can receive clarity within days rather than weeks or even months.
We have previously used the EV blood test in many hundreds of chest pain patients to identify CCS patients. In this project, we have investigated several different methods to automate the method for reliably testing larger numbers of patient samples. We have currently settled on affinity-based capture of EVs with nanochips that are run on an automated platform with the goal to make the technology usable in a regional laboratory facility serving GPs. This will greatly improve the early diagnosis and efficiency of care of these patients, in whom CCS is suspected, and thus reduce healthcare costs and improve prognosis.
