
Using Continuous Glucose Monitoring and contextual data to increase insight in the (individualized) effect of lifestyle on glucose patterns for individuals with type 2 diabetes (Gluco-insight)
Towards more effective T2D self-management using intelligent algorithms for individual glucose optimization via lifestyle changes.
Together, TNO, LUMC, Roche, Ekomenu and Reinier Haga MDC investigated individual causes of (un)healthy glucose patterns in type 2 diabetes (T2D) patients.
Lifestyle modifications by people with T2D have been shown to be an effective way to improve diabetes-related health markers and to reduce medication requirements. Personalized lifestyle recommendations can potentially help to further improve this. However, this can only be implemented once the highly individual glucose response to lifestyle factors can be measured and/or predicted.
To this end, an experimental study to acquire continuous glucose data and contextual data during observational and intervention phases was performed as the core of the project. A total of 41 people with type 2 diabetes was equipped with a continuous glucose monitor that measured their glucose levels while they continued daily life (observational phase) or followed lifestyle interventions such as diet and exercise. Multiple analysis and modelling techniques were used to identify relations between glucose profiles and lifestyle at the individual level. These methods included physiological modelling, multilevel modelling, Bayesian modelling , and machine learning techniques.
The rich dataset of continuous glucose data and contextual data collected in the project provided adequate information to feed computer models that give better insight into the influence of contextual factors (diet, physical activity, sleep) on glucose metabolism on an individual basis. Feature selection methods employed by the modelling algorithms allowed to propose minimal sets of salient features that are key to predicting glucose. The resulting models were shown to allow for more accurate, tailored lifestyle advice. Nine conference publications were realized, several more are planned, and 5 manuscripts are being prepared for submission to high-profile scientific journals.
