Metabolic diseases caused by lifestyle are on the rise, especially in the low social-economic status (SES) population (Sommer et al., 20151). While current standard treatment of these diseases is prescribing symptom lowering medication, mounting evidence shows that changing lifestyle is effective in both prevention and reversal of disease (Lean et al., 20182), but there is a lack of actionable personal health advices especially sticking to lifestyle changes remains challenging. Nowadays the benefits of the internet and digital innovation are permeating into practically every area of our lives and we are amassing vast volumes of data and using many apps. Can’t we use digital techniques to help people in maintaining their health and use health data to fuel them? Thereby we can improve our health and making healthcare more cost effective.

The aim of Connect2Health Consumer was to create and test the architectural design and demonstrate a proof of concept of a data visiting method for health data to bridge and connect data from the healthcare setting and from the consumer/personal setting in a privacy-by-design way. For this aim three aspects were of great importance; 1) the design of the infrastructure that includes data and interoperability standards for the Personal Health Train, 2) mapping of the legal compliance of the architectural design and 3) testing of proof-of-concepts of the infrastructure by conducting two use-cases.
